survey and when you report a problem with our platform, or use some other feature of the
HealthBlocks 360 as available from time to time.

Patient Information: Information entered by healthcare professionals
about their patients, including but not limited to medical history, treatment records,
prescriptions, and test results.

purposes, including, billing addresses, and other payment-related details.
Information we receive from other sources. We may receive personal information

you from individuals or corporate entities which are subscribers to the HealthBlocks 360
(“Subscribers“)
where you are to be designated a user of HealthBlocks 360. We may receive personal information

if you use any of the other websites we operate or the other services we provide from time to
time. We
also work closely with third parties (including, for example, subcontractors in technical,
payment and
delivery services, advertising networks, analytics providers, search information providers,
credit
reference agencies) and may receive information about you from them, subject to your agreements

HealthBlocks 360 and information about the device you use to access HealthBlocks 360.

Personal Information Collector’s filing system, in both live and backup systems.

due to inaccurate, incomplete, outdated, false, unlawfully obtained, or unauthorized use of your
personal data, taking into account any violation of your right and freedoms as data subject.
Right to File a Complaint. If you feel that your personal information has been misused,
maliciously disclosed, or improperly disposed of, or that any of your data privacy rights have
been violated, you have a right to file a complaint with the National Privacy Commission of the
Philippines.
Right to Rectify. As a data subject, you have the right to dispute the inaccuracy or error in
your personal data and have the Personal Information Collector correct the same within a
reasonable period of time.
Right to Data Portability. As a data subject, you have the right to obtain from the Personal
Information Collector a copy of your personal data and/or have the same transmitted from one
Personal Information Collector to another, in an electronic or structured format that is
commonly used.

request that you verify your identity prior to transferring personal information. You may also
access, correct or request deletion of your personal information by logging into your HealthBlocks
260 account. We will respond to your request within 30 days.

We recognize the importance of protecting children's privacy. We collect personal information about
children only with the explicit consent of their parents or guardians. If we become aware that we
have collected personal information from a child without the required consent, we will take steps to
delete such information.
